HIPAA forms and compliance:

As though you had nothing better to do! Again, we apologize for the inconvenience, but we are being required to update information for compliance with HIPAA laws. New AND existing patients will be asked to complete a form on check-in. It won't need to be done again until...next year when they say we'll need to do it again! Once again, if you could arrive a few minutes before your appointment you will have time to complete the form before your visit with your provider. We sincerely appreciate your patience on this issue and certainly share your frustration with what seems like unnecessary time filling out forms. We too would much rather focus on the real reason(s) that you come to see us!
